HomeCare Elements™

A home health care clinical data management and point-of-care data collection system.


HomeCare Elements" is a software application that records patient clinical assesements and visit notes. The assessment and visit information can be recorded in a patient's home using notebook or pen computers. When completed these records are sent to the homecare agency's central computer for storage, data analysis, transmittal of OASIS" required data, and the printing of necessary clinical reports and forms. Other features include:

HomeCare Core" - the comprehensive assessment module documents clinical patient assessments fully integrating the OASIS" data set as prescribed in the Medicare Conditions of Participation. The software consists of two program elements consisting of a central file server program and a remote program. The central file server program is responsible for storing all patient records, making backups, archiving patient records, and coordinating the synchronization of patient records between itself and the remote computers. The remote programs allow for the entry of assessment data in the patient's home or other remote locations using desktop PC's, laptops, workstations on a network, or pen based computers.

The care planning module, including numerous HomeCare Strategies" pathways and VisitLink" visit note tracking system uses information from comprehensive assessment data to assist the clinician to quickly produce an individualized, evidence-based patient care plan. The plan is an integrated, multidisciplinary tool that:

  • Projects costs of the care planned against the agency Beneficiary Cap and projected PPS Payment
  • Identifies and allows prioritization of patient-specific problems
  • Supports maximum outcome attainment with minimum resource use
  • Suggests interventions and activities for each problem identified
  • Meets Joint Commission requirements for care planning
  • Meets the Medicare CoP requirement for interdisciplinary care planning and coordincation

The HomeCare Strategies are similar to clinical pathways in that they direct care toward outcome attainment and optimize the efficiency and effectiveness of service delivery. The program differs from traditional critical pathways, however, in that it bases the plan of action on identified needs for help rather than a list of potential problems extracted from one or more medical diagnoses. This facilitates patient progress and assists the nurse to more accurately identify and track outcome predictors.

VisitLink" uses the care plan to guide each visit into compliance. Activities selected during the planning process serve as prompts to structure the visit note. Each entry is time and date stamped and can be "locked and filed" for security. Notes for all disciplines are included as well as documentation of team communications, physician contacts, orders, and care plan changes.
